التركيب:

أقراص مغلفة تغليفاً معوياً:

يحتوى كل قرص على ديكلوفيناك صوديوم ................... 25أو 50 مجم.

أمبول 3 ملليلتر:

يحتوى كل أمبول على ديكلوفيناك صوديوم .................. 75 مجم.

أقماع:

يحتوى كل قمع على ديكلوفيناك صوديوم .................... 100 مجم.

الخواص:

الرومارين مركب غيرستيرويدى يثبط عملية تخليق البروستاجلاندين المتسبب فى حدوث الالتهابات الروماتيزمية والألام. لذلك فإنه مضاد للإلتهابات ويسكن الألام ويزيل التورم. يكون امتصاص الرومارين امتصاصاً سريعاً وتاماً ويصل إلى أعلى تركيز له فى الدم خلال 2-3 ساعات. ويكون تركيزه فى السائل الزليلى المفصلى مرتفعاً خلال 2-4 ساعات كما يلتصق الرومارين ببروتينات البلازما بنسبة 99% وعند تعاطى الرومارين على هيئة حقن يكون أعلى مستوى له فى الدم مساوياً لضعف أعلى مستوى عند تناول نفس الجرعة عن طريق الفم ولا تتأثر حركيات دواء الرومارين بتأثير وظائف الكبد وتبلغ فترة نصف العمر للرومارين 1-2 ساعة، ويتم إخراجه عن طريق البول وأيضاً عن طريق العصارة الصفراوية.

دواعى الإستخدام:

◄ حالات الروماتويد ، التهابات المفاصل العظمية، التهابات الفقرات الالتصاقية.

◄ حالات النقرس الحادة، روماتويد المفاصل عند الأطفال.

◄ الالتهابات الروماتيزمية المختلفة: التهابات الكيس المفصلى، ألام العضلات الروماتيزمية، التهابات عرق النسا،التهابات أربطة العضلات.

◄ إصابات الأغشية الرخوة: الإلتواء المفصلى، شد الأربطة والعضلات.

◄ حالات عسر الطمث والصداع والصداع النصفى.

◄ اًلام ما بعد الولادة وألام ما بعد الجراحة.

◄ ألام الأسنان، المغص الكلوى والمغص المرارى.

موانع الإستخدام:

◄ القرحة الهضمية النشطة.

◄ الحساسية لمضادات الالتهابات الغير ستيرويدية مثل الربو، الأرتيكاريا أو الإلتهاب الحاد بالأنف و لحساسية للأسبرين.

إحتياطات:

◄ يراعى الحرص عند الاستخدام بالنسبة للمرضى السابق إصابتهم بالقرحة الهضمية.

◄ يكون قرار الاستخدام فى حالة الحمل أو الرضاعة خاضعاً لتقدير الطبيب.

تحذيرات:

تحتوى الأمبولات على مادة سلفيت الصوديوم التى قد تسبب تفاعلات حساسية شديدة مثل الهرش، أزيز الصدر، فرط الحساسية، الأزمة الربوية الشديدة لدى بعض الأشخاص ذوى الحساسية.

الجرعة و طريقة الإستخدام:

◄ عن طريق الفــــم:

(75-150 مجم) مقسمة على 3 جرعات يومياً.

◄ عن طريق الشرج:

100 مجم (لبوسة واحدة) يومياً.

فى الحالات الشديدة يعطى 75 مجم (أمبولة واحدة) بالعضل مرة واحدة أو مرتين يومياً، وفى حالات المغص الكلوى يعطى 75 مجم (أمبولة واحدة) وتكرر مرة أخرى بعد 30 دقيقة.

◄ للأطفال:

الجرعة المقترحة عن طريق الفم فى حالات التهابات المفاصل المزمنة عند الأطفال هى 1-3 مجم لكل كجم من الوزن يومياً مع مراعاة تقسيم الجرعة.

الحفظ:

يحفظ عند درجة حرارة لا تزيد عن 30 ْم.

بعيدأعن ضوء الشمس المباشر.

يحفظ بعيدأ عن متناول الأطفال.

العبوة:

علبة تحتوى على 20 أو 1000 قرص (25 أو50 مجم).

علبة تحتوى على 3 أمبولات أو 100 أمبولة.

علبة تحتوى على 5 أو 6 أقماع.

علبة بها 250 قمع (عبوة مستشفيات).

Rheumarene
(Tablet, ampoules, suppositories)
Composition:
Each enteric coated tablet contains:
Diclofenac sodium 25mg 50 mg

Each 3 ml ampoule contains:
Diclofenac sodium 75 mg
Each suppositery contains:
Diclofenac sodium 100 mg
Properties:
Rheumarene is a non-steroidal anti inflammatory preparation with analgesic and antipyretic properties. It acts by inhibition of prostaglandin synthesis. Rheumarene combats inflammation, relieve pain, tenderness and oedema. Rheumarene is rapidly & almost completely absorbed. Peak plasma concentration is attained after 2-3 hours & significant concentration in the synovial fluid within 2-4 hours. The drug is 99% to plasma protein. Given parent rally the peak blood level is twice that of the same dose administrated orally Hepatic impairment does not affect the pharmacokinetic of Rheumarene. The terminal half life time is 1-2 hours. It is excreted primarily in the form of metabolites mainly in the urine but also in the bile.
Indications:
► Rheumatoid arthritis,osteoarthritis,ankylosing spondylitis.
► Acute gout, juvenile rheumatoid arthritis.
► Various rheumatic diseases : Bursitis, myalgia, sciatica, tendinitis.
► Acute soft tissue injuries: sprain,strain.
► Dysmenorrhea, headache, migraine.
► Postoperative & postpartum pain.
► Dental pain, renal & biliary colics.
Contraindications:
► Active peptic ulcer.
► Hypersensitivity to the drug or to any NSAIDs e.g. asthma, urticaria or acute rhinitis and aspirin sensitivity.
Precautions:
► Use with caution in patients with history of peptic ulcer.
► Use in pregnant & lactating women requires that the benefit of treatment be weighed against possible isks to both mother & child.
Warning: Ampoules contain Sodium sulphite , a sulphite that may cause serious allergic type reactions (e.g.hives, itching, wheezing, anaphylaxis, sever asthma attacks) in certain susceptible persons.
Dosage & route of administration & duration: Adults:
► Orally :75-150mg daily in divided doses.
► Rectally:100mg(one suppository )daily.
► I.M: 75mg ( one ampoule ) once daily & if required in severe conditions 75 mg twice daily.
In renal colics a dose of 75 mg repeated once after 30 minutes is given I.M.
Children:
The suggested dose by mouth for juvenile chronic athritis is 1_3mg/Kg body weight daily in divided doses.
Side effects:
Rheumarene is well tolerated. Side effects are similar to those of other NSAIDS. G.I complaints include epigastric pain,dyspepsia, nausea & vomiting. Skin rashes and minor CNS side effects like dizziness, drowsiness have been reported. Abdominal liver function tests have occurred in rare cases including elevation of transaminase level & liver enzymes.
Storage:
Keep at temperature not exceeding 30ºC
Keep medicine out of reach of children.
