التركيب:
تحتوى كل كبسولة على :
بيروكسيكام ............................................. 20 مجم.
يحتوى كل قمع على :
بيروكسيكام ............................................. 20 مجم.
يحتوى كل 1 مللى أمبول على :
بيروكسيكام ............................................. 20 مجم.
الخواص:
يعتبر البيروكسيكام مركباً غير ستيرويدى مضاد للإلتهابات و له تأثير مسكن و خافض للحرارة .
و يقوم البيروكسيكام بتأثيره عن طريق خطوات متعددة مثل:
تثبيط تصنيع البروستاجلاندين .
تثبيط تجمع خلايا النيتروفيل و البوليمورفز لمنطقة الإلتهاب.
تثبيط إفراز إنزيمات الإلتهابات من خلايا الدم البيضاء.
تقليل إفراز معامل الروماتويد فى السائل السينوفى و يكون إمتصاص المستحضر سريعاً عند التعاطى عن طريق الفم و يصل لأعلى تركيز له فى البلازما بعد 2- 4 ساعات و فترة نصف العمر له تبلغ 45 ساعة، و يكون التصاقه ببروتينات البلازما عالياً و يصل إلى 99 % ويتم إخراج 10 % من المستحضر عن طريق البول بنفس صورته، بينما يتم إخراج نواتج الأيض المختلفة عن طريق البول و البراز.
دواعى الإستعمال:
◄ الالتهابات العظمية المفصلية والأمراض والإصابات العضلية والعظمية وحالات الروماتويد.
◄ الآلام الناشئة عن الإصابات وما بعد التدخلات الجراحية.
◄ عسر الطمث الأولي وحالات النقرس.
◄ للاج إرتفاع درجة الحرارة والألم المصاحبين لإلتهاب الجهاز التنفسى العلوى.
نواهى الإستعمال:
حالات قرحة الأمعاء - الحساسية للأسبرين أو مضادات الإلتهابات الغير ستيرويدية الأخرى - مرضي الربو الشعبي.
تحذيرات:
يستخدم العلاج بحرص مع ذوي التاريخ المرضي لتقرحات الأمعاء أو أمراض الدم أو القلب.
لم يثبت حتى الآن سلامة إستخدامه مع الحوامل أو أثناء الإرضاع.
الأعراض الجانبية:
اضطرابات في الجهاز الهضمي مثل آلام في المعدة أو آلام بالبطن أو غثيان تسبب التوقف عن التعاطي في 5% من الحالات ، وقد يحدث بصورة أقل دوار أو صداع أو ظهور طفح جلدي ، ومثل المستحضرات الأخري المشابهة للأسبرين قد يسبب المستحضر خللاً في وظائف الصفائح الدموية أو ظهور أزمات ربوية مع المرضي ذوي الحساسية للأسبرين.
الجرعـة:
عن طريق الفم:
الالتهابات العظمية المفصلية والروماتويد:
جرعه مبدئية 40 مجم يومياً لمدة يومين (جرعة واحدة أو مقسمة)
جرعات يومية بعد ذلك 20 مجم يومياً لمدة 7 - 14 يوماً.
الأمراض والإصابات العضلية و العظمية:
جرعة مبدئية 20 مجم يومياً .
جرعات يومية بعد ذلك 20 مجم يوميا .
حالات النقرس:
جرعة مبدئية 40 مجم (جرعة واحدة في اليوم الأول) .
جرعات يومية 40 مجم (مقسمة علي جرعتين لمدة 4 - 6 أيام) .
حالات عسر الطمث:
جرعة مبدئية 40 مجم يوميا ولمدة يومين (ويجب بدء العلاج بمجرد ظهور الأعراض) .
جرعه يومية 20 مجم يوميا لمدة 1-3 أيام التالية .
عن طريق الشرج ( باستخدام الأقماع ):
تكون الجرعة مطابقة للجرعات عند التعاطي عن طريق الفم مع دواعي الاستخدامات المختلفة .
عن طريق الحقن بالعضل:
يستخدم الحقن بالعضل في الحالات الحادة والشديدة ويستكمل العلاج بعد ذلك عن طريق الفم أو الشرج.
الحفظ:
يحفظ عند درجة حرارة لا تزيد عن 30ْم.
يحفظ الدواء بعيداً عن متناول يد الأطفال.
العبوة:
علبة بها 10 كبسولات.
علبة بها 6 أقماع.
علبة بها 3 أمبولات (1 مللي).
Feldoral
( Capsules, Ampoules and Suppositories )
Composition:
Each capsule contains:

Properties:
► Feldoral is a non-steroidal anti-inflammatory drug which possesses analgesic and antipyretic properties.
► Feldoral acts at several steps in the immune and inflammation responses through:
► Inhibition of prostaglandins synthesis.
► Inhibition of neutrophil aggregation.
► Inhibition of polymorphonuclear cell migration to the area of inflammation.
► Inhibition of lysosomal enzyme release from stimulated leucocytes.
► Reduction of synovial fluid rheumatoid factor production in patients with seropositive rheumatoid arthritis.
► Feldoral is completely absorbed after oral administration and peak concentration in plasma occurs within 2-4 hours.
► Neither food nor antacids alter the rate or extent of absorption. There is an enterohepatic cycling of piroxicam and the mean value of half life appears to be 45 hours.
► After absorption, Feldoral is extensively (99%) bound to plasma proteins.Less than 10% of the drug is excreted in the urine unchanged. the major
► Metabolic transformation is hydroxylation of the pyridyl ring and this compound and its glucuronide conjugate account for about 60% of the drug excreted in the urine and feces.
Indication:
► Acute musculoskeletal disorders.
► Osteoarthritis.
► Ankylosing spondylitis.
► Rheumatoid arthritis.
► Acute gout.
► Pain after operative intervention and following acute trauma.
► Primary Dysmenorrhea.
► For relief of pain and fever associated with acute upper respiratory tract inflammation.
Dosage:
► Acute musculoskeletal disorders:
Start dose is 40 mg daily for the 1st.two days in single or divided doses.
Maintenance dose is 20 mg daily for 7-14 days
► Osteoarthritis, Ankylosing spondylitis, Rheumatoid arthritis:
The recommended start dose is 20 mg daily.
The maintenance dose is 20 mg daily.
► Acute gout:
start dose is 40 mg daily in a single dose.
Maintenance dose is 40 mg daily in divided dose for 4-6 days.
► Dysmenorrhea:
start dose is 40 mg daily and should be given at the earliest onset of symptoms for the 1st. two days.
Maintenance dose is 20 mg daily for the next 1-3 days.
Side effects:
Gastrointestinal reactions such as epigastric distress, nausea, constipation and abdominal pain are the most common. But less than 5% of patients discontinue treatment because of these. Rare incidence of dizziness, headache and skin rash. as with other aspirin -like drugs, piroxicam alters the function of the platelets and it should be assumed that piroxicam will precipitate bronchoconstriction in patients who are hypersensitive to aspirin.
Contraindications:
Feldoral is contraindicated in:
1- Active peptic ulceration.
2- Hypersensitivity to the drug, aspirin and other NSAIDs.
3- Asthmatic patients.
Precautions
Close control of patients with history of upper gastrointestinal disease or compromised cardiac functions.
Safety during pregnancy and lactation has not been yet established.
